Background & Objectives: In this study, it has been aimed to make a comprehensive comparison on nonintubated patients in anesthesia intensive care unit between CAM-ICU and NEECHAM confusion scales which are accepted as delirium screening tests by basing on an international guide DSM-IV diagnostic criteria. Materials & Methods: One hundred and fifty five adult patients, who were admitted into Pamukkale University Hospital, Anesthesia Intensive Care Units 1 and 2, stayed here for over 24 hours, were nonintubated and not in coma, were over age of 18, and whose Richmond sedation-agitation scale (RAAS) is higher than ≥ -3 and Glaskow coma score is higher than 10 were included in the study. Firstly CAM-ICU confusion test and then NEECHAM confusion test were applied on each patient by the same person at the same hour of the day. Test application periods were recorded. Meanwhile, delirium screening tests were applied on these patients with DSM-IV criteria by a doctor from the department of psychiatry. In the calculation of incidence rates, patients who had a positive value once any test were considered as delirium. Results: According to DSM-IV delirium incidence was identified as %23,2 (n=36) in our intensive care unit. The average age of patients with delirium (68,89±12,7) was found higher (p=0,005) than the average age of patients without delirium (57,38±20,23).The sensitivity of NEECHAM confusion scale over DSM-IV was detected as %88.9, its specificity (selectivity) was detected as %97,5. The Sensitivity of CAM-ICU confusion scale over DSM-IV was found as 0, its specificity was found as %98,3.Mortality rate of patients with delirium was found significantly high (p=0,0001).The application period of CAM-ICU was found shorter (p<0,0001). Conclusion: In this study, it was confirmed that CAM-ICU confusion scale is a more sensitive and specific screening test and could be applied faster than NEECHAM confusion scale in the intensive care unit. It was concluded that the routine usage of these scales are going to be useful.
