Abstract

The delirium is a common complication on intensive care units and can result in prolonged mechanical ventilation or even delay of the whole intensive care treatment. The hyperactive delirium or the hypoactive confusion can be differentiated just with the clinical examination. Promoting factors, such as salt-and water-disturbances, febrile infections, pharmacological agents or the influence of alcohol can be diagnosed easily. Besides, apparative diagnostics include computed tomography, magnetic resonance imaging or electroencephalography, as well as the evaluation of the cerebrospinal fluid. With these, differential diagnoses like subarachnoid-, or intracerebral hemmorrhage, intracerebral abscess, encephalitis syndromes or seizures can be ruled out. Preventive methods include consideration of certain environmental facotrs (for example lighting) and supply of supportive care aids (for exapmle glasses or hearing aids). Regarding the pharmacological treatment of delirium or hypoactive confusion, the etiology of the delirium has to be considered. This includes regulation of the circadian rhythm, symphaticolytic therapy, treatment with neuroleptic drugs or sedation with benzodiacepines.
